School Readiness -
Preschool/Kindergarten Transition Planning

Preschool/Kindergarten Transition Plan Project Description:

To meet the needs identified through the Children Learning Initiative, a School Readiness Transition Plan will be written for each elementary school in the county, facilitated by a Kindergarten Lead and an ECE Lead. The plan will include transition activities for children between the ages of 0-5 years, with at least one elementary school in each district offering a family literacy program for children ages 0-3 years.

Project Goal(s):
To Increase a child’s readiness for school through district/school level transition plans for each elementary district in the county.

  • Identify a Kindergarten Lead teacher for each elementary school/district. Districts may elect to have one Lead for each elementary school or one Lead per district.
  • Identify ECE Lead for each elementary school/district.  Assignment of elementary schools will be based upon the number of eligible applicants.
  • Promote School Readiness activities for each elementary district in the county through the implementation of district/school level School Readiness Transition Plans.
